The main historical data which provoked the creation of COSAM and some elements of the political and social context associated with the development of this project are presented, from the experience of its main manager. The writing is focused on the practical work in the community and its peculiar characteristics, as well as on the official discourse evolution that helps or hinders the development of that activity. The main actors, the working teams, appear with their doubts and conflicts, in an activity that do not permit them escape. In some words emitted by one of those who have to face the interaction with these types of residents “I don’t even have my own place to hide in”, referring to his job in hospitals or in the Ministry of Health facilities.
